Family Law Financial Affidavit - Over $50,000: This affidavit is required when one of the individuals in the divorce receives $50,000 or more in Gross Annual Income.
Parenting Plan: A Parenting Plan is required in all cases involving time-sharing with minor child(ren), even when time-sharing is not in dispute. The Parenting Plan must be developed and agreed to by the parents and approved by the court.
Mandatory Disclosure: Each party in a dissolution of marriage case is required to provide the other party with certain financial information and documents. Under Florida Law, we must receive the documents requested in the form in our office within 14 days of your receipt of notification that this form is required.
Interrogatories: This form should be used to ask the other party in your case to answer certain standard questions in writing. The questions in this form should be used in original proceedings or enforcement proceedings and are meant to supplement the information provided in the Financial Affidavit.
Uniform Child Custody Jurisdiction and Enforcement Affidavit: This form should be used in any case involving custody of, visitation with, or time-sharing with any minor child(ren). This affidavit is required even if the custody of, visitation, or time-sharing with the minor child(ren) are not in dispute.
